Information about you is used for the following purposes:

• To deliver safe and efficient client care
• To assess and provide for your therapeutic needs
• To advise you of alternate treatment options
• To establish and maintain communication
• To efficiently follow up treatment, care and billing
• To complete and submit claims for the cost of treatment to a third party
• To comply with regulatory requirements, including the College of Massage Therapists of Ontario, who may inspect our records as part of their regulatory activities in the public interest
• To collect unpaid accounts
• To comply generally with the law

A charge of $70 will be applied to all missed/non-cancelled appointments.  Your benefits do not pay for missed or non-cancelled appointments; this payment must come directly from you.  Your appointment time is reserved just for you.  If you are unable to keep your appointment, please give 24 hours notice.
